LIFE INSURANCE

You will receive term life insurance equal to one-and-one half times your annual base salary (minimum of $35,000) up to a maximum of $50,000. There is no cost to you for this coverage. The effective date for life insurance is the first day of the month following 90 days of employment. You may purchase additional coverage for yourself, your spouse, or your dependent children, which may be subject to underwriting approval. You will be asked to complete a beneficiary designation form at your benefits orientation. Hartford Life Insurance Company is our life insurance provider.
